Title : Letter and travel declaration

Dear Students

Please download the letter and travel declaration form and show them to your parents. (Log on to IVLE, go to School Information Module - workbin)

If you have travelled to an H1N1 affected country in the last 7 days, please submit the travel declaration form via email to rvhs@moe.edu.sg immediately. You will be given Leave of Absence and are to stay away from school for 7 days (excluding the day of return) after returning from the affected country.

If you have NOT travelled to an H1N1 affected country in the last 7 days, please submit the travel declaration form on the first day of school to your form teachers.

Please seek medication attention if you are not feeling well. Rest well at home, and be socially responsible by not going out to the public areas.

Your form teacher will contact you via phone to ask for some information. Please also remember to bring along your Oral Digital Thermometer everyday to school for temperature taking.

If you forgot your IVLE password, please email to rvhs@moe.edu.sg stating your name, IC number, class and phone contact. We will reset your password and keep you updated.
